Use this blank match report in cases where you may have lost your original. Be sure to print 3 copies and input the match number and other information correctly.
Use this RAINOUT form in situations where your match has been rescheduled due to fields closing for rain or inclimate weather. No pre-approval is needed for this form.
This PART A needs to be completed and sent to firstname.lastname@example.org or via fax to 856-0204 for approval before proceeding to PART B in the reschedule process.
By submitting PART B, you are confirming that both teams have OKed the reschedule and that you have contacted your field coordinator and referee assignor to assure that the field listed is available and referees will be present.
