Team Check In Instructions:

To ease travel burdens and futher cater to our tournament teams we offer online check-in.  Please email an electronic copy of the following documents to tournaments@thecrew.com no later than September 15th:

  1. Approved Roster 
  2. Validated Player Pass Cards, both sides for both rostered players and guest players 
  3. Validated Coaches Pass Card (maximum of 3 coaches)
  4. Concussion Training Certificate (click here for out of state coaches that need to receive this certification)
  5. Completed Tournament Liability Form which must list all players, including guest players, and be signed by a parent or guardian (click here to download form)
  6. Approved Travel Permit if outside of Ohio South (except for US Club Soccer)
  7. Guest Player Roster approved and validated by the state in which the player is registered (maximum of 4 guest players)
